As Lovecraft invented a ton of alien deitys/monsters it could be one of the other ones.
Remember to not make it a bulletsponge, like in any other 'modern' FPS.
Use the Zelda formula: Do some shit 3 times to kill the boss: e.g.: press 3 buttons to grille some dude in his jacuzzi because of some conveniently placed electrodes.
I wouldn't enjoy an original boss remade.
I don't have a clue how long it takes to make enemys for quake from scratch.

I so much hope this episode becomes real thing!
The guys took some time off for the other jam, but now that it's finished they should be back feeling a little more refreshed.
God I hope not.
Scope creep has meant we've exceeded the initial target date by a long long way. It's creeping ever closer to completion, but I really would like it done soon. The guys are working though ^_^
It's great! Sometimes less is more, quiet is loud...
I am (still) working on a trailer for Jam9 and wanted to warn you the beginning of mine is a bit similar to this video. Didn't want anyone out there going "ripoff!!" But great minds right? :)

The theme is a vanilla-like episode for quake. In the discussion thread I mentioned a possible map flow of the following:
M1 Base map (as per id convention) ->
M2 polluting into a swamp map ->
M3 entering into a ruins map ->
M4 which enters a lost "lovecraftian" city ->
M5 which has a maze like cultist church ->
M6 that has a boss battle.
Secret map -> whatever really.
Market research shows the target audience has a near 100% approval rating for the suggested theme and concept, with the slight proviso that "metal" maps are included as suitable, along with a possible extension to M7.
We've departed from the vanilla feel a little bit, but I'm confident you'll still like it.
The episode flow is almost identical to what you quoted still.
